Form 1095-C for the 2025 tax year is available upon request.

Under the Affordable Care Act (ACA), The City of East Ridge is required to provide Form 1095-C to certain employees to report information about health insurance coverage offered. While this form is for your records and is not required to file your personal income tax return, you may request a copy.

How to Request Your Form:
You may request a copy of your Form 1095-C by contacting us through any of the following methods:

Fulfillment Timing:
We will provide your requested form within 30 days of receiving your request.
